SAMPLE ACCREDITATION APPLICATION

(Full name of the mass media in Ukrainian, English, and the language of the country of origin) requests you to issue accreditation cards to cover the work of The Verkhovna Rada of Ukraine of ___ convocation.

 

address of media central editorial board;

 

address of media correspondent in Ukraine and e-mail (if available);

 

last name, first name, and patronymic name of the journalist (journalists), technical personnel member (members) of the mass media to use accreditation cards.

 

Attached to the application shall be the copies of identity cards the journalist (journalists), technical personnel member (members) of the mass media (passport, editorial card or press card), and two photographs 30х40 mm of each journalist or technical personnel member to use accreditation cards.

Attached to the application shall be the copies of identity cards and professional credentials (passport, education certificate, editorial card, document issued by the professional association of the journalists, press card) and two photographs 30х40 mm of the applicant.

 

Journalists or technical personnel who are permanent or non-permanent employees of media can be accredited on application by the media they cooperate with (in line with sub-items 8.1-8.5 of the Ordinance concerning Accreditation of Journalists and Technical Media Personnel in the Verkhovna Rada of Ukraine).
